  1. Prepare the loss documents

    Obtain a notarized affidavit; add a police report if the lost passport is valid.

  2. Check the documents before booking

    Check that your birth record and ID details agree; arrange missing PSA documents first.

  3. Choose a DFA office and appointment

    Select office, date and processing option at passport.gov.ph/appointment.

  4. Pay using the emailed reference

    Follow the deadline in the payment email and keep the receipt.

  5. Print the confirmed packet

    After payment, print the confirmed appointment and completed application on A4 paper.

  6. Attend document checking and biometrics

    Appear personally at the office on your booking; bring originals and photocopies.

  7. Collect and inspect the passport

    Follow the release date on your receipt; check the printed details. Delivery adds time.

Print the packet emailed after payment; bring the payment receipt.

PSA birth certificate or Report of Birth

Bring the original and a photocopy. Unreadable, corrected or late-registered records require extra documents.

Examples include an LTO driver’s licence, PRC ID or UMID. Bring the original plus one photocopy.

PSA marriage certificate when using a spouse’s surname

Bring the original and a photocopy when changing to a married surname; this is not needed solely for retaining a maiden name.

Notarized affidavit of loss

Required for a lost valid or expired passport.

The DFA FAQ does not require a police report for loss of an expired passport.
