- Check the documents before booking
Check that your birth record and ID details agree; arrange missing PSA documents first.
- Choose a DFA office and appointment
Select office, date and processing option at passport.gov.ph/appointment.
- Pay using the emailed reference
Follow the deadline in the payment email and keep the receipt.
- Print the confirmed packet
After payment, print the confirmed appointment and completed application on A4 paper.
- Attend document checking and biometrics
Appear personally at the office on your booking; bring originals and photocopies.
- Collect and inspect the passport
Follow the release date on your receipt; check the printed details. Delivery adds time.

What you’ll need
Print the packet emailed after payment; bring the payment receipt.
Bring the original and a photocopy. Unreadable, corrected or late-registered records require extra documents.
Examples include an LTO driver’s licence, PRC ID or UMID. Bring the original plus one photocopy.
Bring the original and a photocopy when changing to a married surname; this is not needed solely for retaining a maiden name.
