What you’ll need
Use your existing SS number; enroll a disbursement account through DAEM when filing directly.
Include diagnosis, recommended leave, clinic contact and physician licence number; attach relevant test or hospital records.
Previously employed direct claimants may need proof of separation and whether the employer advanced the benefit.
Steps
At least 3 paid contributions in the 12 months before the sickness semester; employed applicants generally must exhaust paid sick leave.
Official serviceHome confinement: notify the employer or SSS within 5 calendar days. Hospital claims have different deadlines. Late notification can reduce or deny the claim.
Official serviceEmployees submit medical records to their employer. Direct applicants use My.SSS → Benefits → Sickness Benefit and upload records; exceptional claims need a branch.
