• Personal email and mobile number

  Use contact details you can access for the confirmation link and later My.SSS registration.

• Birth certificate or SSS-accepted alternative identity record

  Prepare a clear scan. The official registration page lists alternatives when a birth certificate is unavailable; use the matching combination, not an arbitrary ID.

• Spouse / children’s civil-registry records, if declaring them

  Use a marriage certificate for a spouse and a birth / adoption record for a child. Keep the originals for validation.

   Enter your civil-registry details accurately and follow the emailed link within five calendar days.

   Open the official service

   Upload supporting documents and retain the registration record

   Save the issued SS number and registration forms. Submit / present any documents SSS requests to convert the record from temporary to permanent.

   Register My.SSS and check the correct contribution category

   Employees give the number to the employer. Self-employed and other individual payors use the applicable contribution schedule.
