What you’ll need
Form 90, birth/baptismal certificates, PSA CENOMAR for each, one 2×2 photo each and valid IDs; residency/tax proof where needed.
Family-planning certificate; ages 18–20 need parental consent, 21–25 parental advice, 18–25 marriage counselling. Foreign or previously married applicants need additional civil-status records.
Steps
QC e-Services → Civil Registry Online Services → Marriage License Application; fill both applicants’ details and upload records.
Official serviceFollow the email; both parties appear at the City Civil Registry and pay at the City Treasurer.
Official serviceReturn after 11 days with the notarized application and required certificates. QC lists filing/release until 3 pm.
Official serviceUse the issued licence within 120 days. This guide follows the QC process and is not a nationwide fee schedule.
