A PhilHealth Local Health Insurance Office (LHIO). Employees can submit through the employer’s enrollment process; ask the LHIO before sending personal records by email.

What to bring

PhilHealth Member Registration Form (PMRF)
Tick FOR UPDATING and write the existing PIN. Mark the exact entries or dependents to change.
Identity and relationship evidence
Bring accepted identity evidence. For a spouse use the marriage certificate; for children use birth / adoption records. A correction needs the civil record supporting the new entry.
Existing PIN and Member Data Record (MDR)
Bring the record showing the entry you want to correct. Do not create a second PIN.
