Passport Renewal · Changing my name

Add original PSA-issued records supporting the change. Marriage is not automatically a requirement when retaining your maiden name.

Steps

  1. Choose your renewal case

    Review your selected case in the guide. Lost, damaged, older-passport and minor cases require additional documents.

  2. Book your DFA office and pay

    Use passport.gov.ph. Pick the actual office and processing option; follow the payment deadline in your booking email.

  3. Prepare originals and photocopies

    Bring the original ePassport/data-page copy, confirmed application and original PSA-issued records supporting the name change.

  4. Attend your appointment

    Go to the DFA office stated in your booking for validation and biometrics.

  5. Collect or receive your passport

    Use the release date on the official receipt. Check your name and details; delivery takes additional time.
