What to bring

Valid ID of the person requesting assistance
Prepare a current ID with photo and matching name.
Recent medical record and expense document
Bring a medical abstract/certificate or applicable discharge record, normally issued within 3 months. Add the hospital balance, prescription or procedure request for the assistance sought.

Submit records and attend assessment
Present the expense documents and explain the crisis to the social worker; supply any case-specific documents requested.

Retain the decision and receipt; a guarantee letter is used with an approved provider and is not cash deposited to your account.
