Prepare a current ID with photo and matching name.
Bring death certificate or accepted certification, funeral contract or account statement; transfer permit for transport and balance certificate/promissory note if unpaid.

Use a DSWD Crisis Intervention office or satellite desk; hospital cases can start with the medical social-service or Malasakit desk.
